Account signup (test sink)

The form below mimics a generic web-app signup with the common field set. Submission is accepted, a session cookie (__session) is issued, and the browser is 302-redirected to / — exactly like a real signup flow. No account is created in any real system. To verify the submission reached the server, check your browser's network tab.
